The Behavioral Health Navigator (BHN) II provides crisis intervention and de-escalation services to clients and families. Care Manager / Social Worker BACHELORS, Aberdeen

The Care Manager is an integral member of the interdisciplinary team involved with each individual's care at CPC. The Care Manager coordinates services and establishes goals in collaboration with clients/consumers and their support network to promote wellness, recovery, and productive lives. The Care Manager completes clinical documentation, utilize evidence-based practices, provide crisis intervention and link individuals to community support. The Care Manager collaborates with clients/consumers, their support network, the treatment team, and other community providers to ensure positive outcomes to facilitate success at home, in school, at work and in the community. 

 

Requirements: Valid NJ Driver's License; BA Degree in Social Science or other health related field; one year of experience with population served by program Preferred: BA Degree in Social Science or other health related field; two years of experience working with population served by program; CADC

The Behavioral Health Clinician I provides individual, group, family, and/or substance abuse treatment for consumers who are diagnosed with co-occurring, mental health and substance abuse disorders. Works with a team to complete individual recovery planning and provides case management services. Completes Bio psychosocial Assessments, treatment plans, and documents treatment progress in electronic clinical record. May be assigned other duties as necessary.

Requirements: Must possess a masters degree and LSW or LAC. LCADC is preferred.  Knowledge of evidence based practices such as DBT, CBT and MET. A valid NJ driver's license with a satisfactory driving history is required.

Crisis Response Social Worker, Children & Youth MASTERS
Neptune, NJ

The Crisis Intervention Specialist works within a team of professionals to deliver Crisis Response services 24 hours a day, 7 days a week, and provides specific services and interventions for up to 72 hours. You will be required to work a combination of day and evening shift hours and in some instance's weekends. Complete an initial assessment of the client's needs based on program requirements and develop and Individual Crisis Plan to improve the overall functioning and mental health of the child and family. Complete assessments as required by the regulatory agency which governs their program. Re-evaluate the client throughout treatment to identify any new needs or problem areas.  The CIS will make at least weekly calls to the client, which are documented in a timely manner.

Requirements: Master's Degree plus related internship or prior experience providing treatment services to children or adolescents is required; valid NJ Driver's License with a clean driving record. Preferred: Human Service oriented master's degree plus experience providing treatment services to children or adolescents is preferred. 

Spanish or second language skills a plus! An LSW or a LAC is also a plus.

School Social Worker,

The school social worker provides individual, group and family therapy to students with emotional, behavioral and social difficulties that necessitate the need for out of district educational placement. High Point school is a private special education program for students who experience both academic challenges as well as mental health struggles.  The school social worker should have experience in evidence based treatment practices with difficult to engage child and adolescent populations who experience behavioral and mental health struggles.

Requirements: Must have Master's degree in social work from an accredited institution, 2 years of experience with similar population, NJ Dept. of Education School Social Worker Certificate and LSW, with LCSW eligibility within 2 years.  An LCSW with 2 or more years' experience is preferred. Bilingual and/or substance abuse experience a plus.

The Reading interventionist is hired by the Director and administrative team; is fully certified and trained to bring special expertise in the diagnosis, instructional techniques, and assessment specific to reading.  The Reading Interventionist recognizes the need for sensitivity and structure in the education of students with various learning and behavioral disabilities.

PACT Team Supervisor

The Program for Assertive Community Treatment (PACT) is a team-based program for individuals with serious and persistent mental illness and substance use disorders who have a history of multiple emergency room visits or psychiatric hospital admissions in the last 18 months.? CPC's PACT Teams include a psychiatrist, nurses, clinical social workers, co-occurring specialists, vocational specialists, mental health advocates and peer specialists. The PACT Teams provide intensive, in-community treatment and support to assist consumers in staying out of the hospital and improving their functioning in the community. The PACT Team treats adult consumers throughout Monmouth County. Referrals for PACT services can be made directly to the PACT program and must be approved by DMHAS (the Division of Mental Health and Addiction Services). PACT treatment is based on a nationwide evidence-based treatment model.

Our Red Bank-Middletown office is located in the heart of the city, convenient to all major roadways. In this role, you will work closely with other team members to manage schedules for medical doctors and Advanced Practice Nurses (APNs) in a fast-paced environment.

The hours for this role are as follows:

Monday & Wednesday, 1PM to 9PM; Tuesday & Thursday 12PM to 8PM; Friday, 9AM to 5PM.

As an Administrative Assistant, you will manage schedules for the medical staff. Maintenance / Facilities Custodian

CPC Behavioral Healthcare is currently looking for an experienced Maintenance/Custodial Worker to join our Facilities Department.  Our successful candidate will work as part of a team to perform all related buildings and grounds maintenance tasks to create a safe environment for staff, clients and the public across multiple facilities in Monmouth County.

​

Responsibilities include performing routine and preventative maintenance on buildings, grounds and equipment, including painting, plumbing, electrical, carpentry, etc. Performing routine housekeeping/janitorial services throughout the school day. Responsible to perform prescribed  routine cleaning of high touch areas in the school building continuously during the school day to maintain a safe and sanitary environment for staff and students. This position will also help in grounds maintenance, particularly during weather emergencies as well as moving equipment and material between offices and buildings as needed.
