PROGRAMS FOR ADULTS

At CPC we provide highly specialized care for adults with co-occurring mental health and substance use disorders. Our services are delivered with dignity and respect, and we meet each person where they are in their mental health and recovery journey. It is vital to our evidence-based practices, that we make space for hope, healing and the promise of new possibilities.

Individual, Group & Family Therapy

Adult individual therapy for mental health and/or co-occurring substance use disorders is available in Aberdeen, Freehold, Howell and Red Bank. Sessions are typically every other week and can take place in combination with psychiatric services.

Individual therapy or counseling is when a person speaks with a trained therapist in a safe and confidential environment to explore and understand feelings and behaviors and gain coping skills. Individual therapy is effective for both mental health and substance abuse issues.​ At CPC, individuals can meet with licensed social workers and counselors to process their emotions, work toward their goals and improve their lives.

Adult Intensive Outpatient Treatment (IOP)

Adult IOP is three hours daily, three days per week and is offered during daytime or evening hours (depending on location). Treatment typically lasts 12-16 weeks. Transportation is provided for those with Medicaid and we can access special grant funding to assist others with transportation on a case by case basis. Please call to inquire.

This program is a substance use treatment program for individuals seeking a stable lifestyle free from substance use or dependence through the development of healthy coping skills and supports.​ Our Adult IOP specializes in providing treatment for individuals with co-occurring mental health and substance use disorders.​ ​

Medication Assisted Treatment

Initial Medication Assisted Treatment appointments are offered at our Red Bank location with follow-up appointments available at our Aberdeen, Freehold, Howell and Red Bank locations.

We are certified to offer Medication Assisted Treatment (MAT) for opioid use disorder.  MAT is a way to help treat substance use disorders using prescribed medications to help with withdrawal symptoms and cravings. CPC's MAT program addresses substance use in addition to opioids. For example, clients can receive Vivitrol for alcohol use disorders. MAT is currently offered only in combination with substance use counseling services. Situated within two of our counseling offices, customized pharmacy care is available. This one-stop location for coordinated care helps facilitate medication schedules and provides supportive guidance while eliminating transportation barriers.​

CPC currently offers the following prescribed medications as part of our MAT program:

  • Suboxone

  • Vivitrol

  • Naltrexone

  • Campral

Substance Use Group Therapy

Group therapy for substance use disorder takes place weekly and is available at Aberdeen, Freehold, Howell and Red Bank. 

​At CPC, groups meet with licensed social workers and counselors to process their emotions, work toward their goals and improve their lives. Group therapy includes people with similar issues meeting together for counseling and mutual support. Group Therapy takes place often in combination with individual therapy. Substance Use Groups also focus on co-occurring mental health issues.

Group Therapy treatment includes: 

  • Recovery Group

  • Emotional regulation and mindfulness

  • Processing and managing trauma

  • Young Adult group (ages 18-25)

Adult Partial Care

Adult Partial Care is provided at our Aberdeen office. Transportation is provided within Monmouth County.

​Partial care is for people with psychiatric or co-occurring disorders who require an intensive level of care based on presenting issues. The goal is for the individual to remain in his or her home setting while gaining skills to lead an independent and productive life in the community. Meals are provided and evening programming is available.

Psychiatric and Medication Management

Services are available at locations in Aberdeen, Freehold, Howell and Red Bank.

People seek psychiatric help for many reasons. The problems can be sudden, such as a panic attack, thoughts of suicide, or frightening hallucinations including hearing "voices." Or they may be more long-term, such as feelings of sadness, hopelessness, or anxiousness that never seem to lift or problems functioning, causing everyday life to feel distorted or out of control. CPC's psychiatrists and nurse practitioners provide services to diagnose and treat mental, emotional, behavioral and substance use disorders in children and adults.

​PACT Program

The Program for Assertive Community Treatment (PACT) is a team-based program for individuals with serious and persistent mental illness and substance use disorders who have a history of multiple emergency room visits or psychiatric hospital admissions in the last 18 months.​ CPC's PACT Teams include a psychiatrist, nurses, clinical social workers, co-occurring specialists, vocational specialists, mental health advocates and peer specialists. The PACT Teams provide intensive, in-community treatment and support to assist consumers in staying out of the hospital and improving their functioning in the community.​ The PACT Team treats adult consumers throughout Monmouth County. Referrals for PACT services can be made directly to the PACT program and must be approved by DMHAS (the New Jersey Division of Mental Health and Addiction Services).​ PACT treatment is based on a nationwide evidence-based treatment model.

Behavioral Health Home

This program is located at our Aberdeen location and is available to adults 18 and older who are enrolled in Medicaid. The Behavioral Health Home (BHH) is a program for individuals with co-occurring mental health, physical health and/or substance use disorders. Individuals in the Behavioral Health Home program are provided with care coordination services to assist in making medical appointments, following up on recommendations and improving health and wellness.

 

Supportive Employment

Supportive Employment assists consumers in their goal to return and remain in the work force. CPC’s Supported Employment Program offers a continuum of vocational services and emphasizes hands-on training on the job .

  • Career Exploration

  • Problem Solving Skills

  • Travel Training

  • Job Development

Supportive Housing - Community Support Services

CPC offers Supportive Housing throughout Monmouth County.

The scope and purpose of the program is to increase independence, community integration and quality of life.

​

Services include:

  • Completion of comprehensive rehabilitation needs assessment for consumer-directed treatment

  • Development of individualized plan for recovery and wellness

 

Skills development training and skill teaching based on:

  • Daily living skills

  • Social skills

  • Navigating the physical and mental health systems

  • Budgeting, financial stability and entitlements

  • Recreation and leisure

  • Self-advocacy

  • Wellness and a healthy lifestyle

  • Employment, volunteer and educational readiness

  • Coordination of care with individual's family or support system

  • In-community care management

  • Crisis Intervention

  • Supportive counseling

  • Family psychoeducation and support​

Integrated Case Management Services

Integrated Case Management Services (ICMS) assists individuals 18 and over with serious and persistent mental illness and/or co-occurring mental health & substance use disorders with re-integration into the community. Care management services provide linkages to help individuals manage their illness and secure necessary services and resources to improve their quality of life.

​

Services include:​

  • Individualized treatment services in the consumer's home environmental for up to 12 months

  • Assessment/Service Planning

  • Linkage to community services

  • Coordination of care with family and supports

  • Specialized assistance for individuals involved in the justice system 

 

Justice Involved Services

Justice Involved Services (JIS) is a community case management program specific to the justice-involved population. 

Care management services provide linkages to help individuals manage their illness and

secure necessary services and resources to improve their quality of life.

Services include:​

  • Individualized treatment services in the home environment for up to 12 months

  • Assessment/Service Planning

  • Linkage to community services

  • Coordination of care with family and supports

  • Specialized assistance for individuals involved in the justice system
